What 'disavow' actually does
The Disavow tool, launched on October 16, 2012, lets a site tell Google to ignore specified inbound links when computing the site's ranking signals. The instruction is one-way: the submitting site is the only consumer. The links remain on the referring pages; Google's link graph still records their existence; only the ranking weight for the submitting site is zero.
The tool is designed for cases where a site cannot get manipulative inbound links removed through outreach. Disavow against what could not be removed is the operational role.
What disavow does not do
Disavow does not remove a manual action by itself. Google rejects reconsideration requests where the disavow ran without documented removal outreach evidence. The audit trail Google looks for is dated removal requests sent to the referring sites with responses (or non-responses) recorded, then the disavow against the residual, then the reconsideration submission. Skipping outreach makes the disavow read as evading the work rather than executing the remediation.
Disavow does not affect outbound links from your site. It is an inbound-link instrument. Outbound attribution sits separately at NoFollow / Sponsored / UGC.
Disavow does not remove the links from the web or change what Google indexes about the referring pages. It only changes how the submitting site is evaluated against them.
When to file disavow, when it is largely redundant
